Principles and Guidelines for Sharing Biomedical Data for Secondary Use: The University of Chicago Perspective

Academic medical centers are generating an increasing amount of biomedical data and there is an increasing demand for biomedical data for research purposes by research projects, research consortia, companies, and other third parties. At the same time, as the number of patients grows and the amount of data per patient grows, there is an increasing possibility that some information about some patients may become available if the data is shared with third parties and the third parties have a data breach or violate the terms of the data use agreement. Balancing the importance of research that may result in improved patient outcomes with the importance of protecting patient data is challenging. The article discusses the principles, considerations about risks and mitigating risks, and guidelines used at the University of Chicago used for making decisions about sharing biomedical data with third parties.

A network-based biomarkers discovery of Cold/Hot ZHENG chronic gastritis and Cold/Hot herbs of formulae

Objective: To discover biomarkers and uncover the mechanism of Cold/Hot ZHENG (syndrome in traditional Chinese medicine) chronic gastritis (CG) and Cold/Hot herbs in traditional Chinese medicine (TCM) formulae on systematic biology. Background: CG is a common inflammatory disease and the diagnosis of CG in TCM can be classified into Cold ZHENG (Asthenic Cold) and Hot ZHENG (Excess Hot). However, the molecular features of Cold/Hot ZHENG in CG and the mechanism of Cold/Hot herbs in formulae for CG remained unclear. Methods: Based on data of 35 patients of Cold/Hot ZHENG CG and 3 scRNA-seq CG samples, we conduct analysis with transcriptomics datasets and algorithms, to discover biomarkers for Cold/Hot ZHENG CG. And we collected 25 formulae (with traditional effects related to Cold/Hot ZHENG) for CG and corresponding 89 Cold/Hot herbs (including Warm/Cool herbs) to discover features and construct target networks of Cold/Hot herbs on the basis of network target and enrichment analysis. Results: Biomarkers of Cold/Hot ZHENG CG represented by CCL2 and LEP suggested that Hot ZHENG CG might be characterized by over-inflammation and exuberant metabolism, and Cold ZHENG CG showed a trend of suppression in immune regulation and energy metabolism. And biomarkers of Cold/Hot ZHENG showed also significant changes in the progression of gastric cancer. And biomarkers and pathways of Hot herbs intend to regulate immune responses and energy metabolism, while those of Cold herbs were likely to participate in anti-inflammation effect. Conclusion: In this study, we found that the biomarkers and mechanism of Cold/Hot ZHENG CG and those of Cold/Hot herbs were closely related to the regulation of immune and metabolisms. These findings may reflect the mechanism, build bridges between multiple views of Cold/Hot ZHENG and Cold/Hot herbs, and provide a research paradigm for further achieving precision TCM.

Estimation of age-specific excess mortality of men and women with rheumatoid arthritis (RA) in Germany

A MCMC approach is used to estimate the age-specific mortality rate ratio for German men and women with RA. For constructing priors, we calculate a range of admissible values from prevalence and incidence data based on about 60 million people in Germany. Using these priors, MCMC mimics and compares estimated mortality to the findings of a recent register study from Denmark. It is estimated that the mortality rate ratio is highest in the young ages (4.0 and 3.5 for men and women aged 17.5 years, respectively) and declines towards higher ages (1.0 and 1.2 for men and women aged 92.5 years, respectively). The lengths of the credibility intervals decrease from younger towards older ages.

Development of Pipetting Devices to Separate Protein Complexes

The objective of this project is to develop an automated device used to spot protein samples on a hydrophobic membrane to be used for the patented electrophoresis method developed by Chang and Yonan in 2008 [1]. This novel method performs electrophoresis directly on hydrophobic blot membranes as opposed to the previous popular methods such as the 2-D polyacrylamide gel method [2, 3]. This new electrophoresis method significantly reduces the processing time to about 40 minutes, as opposed to the 2-D PAGE method which can take one or two days. Special methods to spot samples on hydrophobic blot membranes were established for successful separation of protein and protein complexes. These spotting methods are used in conjunction with the patented electrophoresis method [1] for effective separation. The automated device effectively replicates these special methods that are repeatable and user independent. The goal of the automated device is to effectively spot and separate protein complexes in addition to single proteins under non-denaturing conditions, thus eliminating the variability of the manual spotting method.

Semi-Quantitative Analysis and Serological Evidence of Hepatitis A Virus IgG Antibody among children in Rumuewhor, Emuoha, Rivers State, Nigeria

Hepatitis A virus (HAV) infection has been greatly reduced in most developed countries through the use of vaccine and improved hygienic conditions. However, the magnitude of the problem is underestimated and there are no well-established Hepatitis A virus prevention and control strategies in Nigeria. The aim of this study was to determine the prevalence of Hepatitis A virus infection among children aged 2 to 9 years in Rumuewhor, Emuoha LGA, Rivers State, Nigeria. Blood samples were collected from the 89 children enrolled in this study, and analyzed for the presence of HAV IgG antibodies using ELISA techniques. Of the 89 participants, 22 (24.7%) tested positive for HAV IgG antibodies, while 67 (75.3%) were negative. The children within the ages of 4 to 6 years had the highest seropositivity rate (33.3%) while those less than 4 years had the least seropositivity rate (22.4%). The prevalence rate ratio of the males to females was 1:1.3. There was no significant difference (p between IgG seropositivity and age groups and gender. However, there was a statistical association of IgG seropositivity rates with respect to immunization. The seroprevalence rate recorded in this study was significant, indicating that the virus is endemic in this study area. Proper awareness, health education and vaccination are imperative to controlling and preventing HAV infection in Rumuewhor, Emuoha, Rivers State, Nigeria.

Emergence simulation of cell-like morphologies with evolutionary potential by virtual molecular interactions

This study explores the emergence of life through a simulation model approach. The model "Multi-set chemical lattice model" is a model that allows virtual molecules of multiple types to be placed in each lattice cell on a two-dimensional space. This model is capable of describing a wide variety of states and interactions in a limited number of lattice cell spaces, such as diffusion, chemical reaction, and polymerization of virtual molecules. This model is also capable of describing a wide variety of states and interactions even in the limited lattice cell space of 100 x 100 cells. Furthermore it was considered energy metabolism and energy resources environment. It was able to reproduce the "evolution" in which a certain cell-like shapes adapted to the environment survives under conditions of decreasing amounts of energy resources in the environment. This enabled the emergence of cell-like shapes with the four minimum cellular requirements: boundary, metabolism, replication, and evolution, based solely on the interaction of virtual molecules.

Green synthesis of silver nanoparticles using Curcuma longa flower extract and antibacterial activity

Silver nanoparticles (AgNP's) possess inherent biological potentials that have obliged an alternative, eco-friendly, sustainable approach to "Green Synthesis." In the present study, we synthesized Green Silver Nanoparticles (GAgNP's) using Curcuma longa L. (C. longa) flower extract as a reducing and capping agent. The synthesized GAgNP's were characterized using UV-Visible spectroscopy, X-ray diffraction (XRD), and High-resolution transmission electron microscopy (HR-TEM), which confirmed their homogeneity and physical characteristics. The GAgNP's were found to contain crystalline silver through XRD, and the particles were confirmed to be homogeneous and spherical with a size of approximately 5 nm, as evidenced by UV-Visible spectroscopy, XRD, and HR-TEM. In addition, the biological potential of GAgNP's was evaluated for their antibacterial activities. GAgNP's showed significant activity and formed different sizes of inhibition zones against all selected bacteria: Mycobacterium smegmatis (M. smegmatis) (26 mm), Mycobacterium phlei (M. phlei), and Staphylococcus aureus (S. aureus) (22 mm), Staphylococcus epidermidis (S. epidermidis) and Klebsiella pneumoniae (K. pneumoniae) (18 mm), and Escherichia coli (E. coli) (13 mm). The MIC value of GAgNP's was found to be between 625 ug/mL-39.06 ug/mL for different microbes tested. With further research, the green synthesis of GAgNP's using C. longa flower extracts could lead to the development of effective antibacterial treatments in the medical field.

Starker Effekt von Schnelltests (Strong effect of rapid tests)

This article is a reproduction of a Fraunhofer ITWM report from 28 June 2021 on the contribution of various non-pharmaceutical measures in breaking the 3rd Corona wave in Germany. The main finding is that testing contributed more to the containment of the pandemic in this phase than vaccination or contact restrictions. The analysis is based on a new epidemiological cohort model that represents testing, vaccination and contact restrictions by time-varying rates of detection, vaccination and contacts, respectively. Only the effectiveness of different vaccines is taken from the literature. All other parameters are automatically identified in such a way that the simulated and the published incidences and death rates match. Among these parameters are incubation time, mean duration of the infectious phase, mortality rate, as well as two contact rates and one detection rate per week. Note that we can reconstruct such a high number of parameters only because we assume that the weekly wave patterns in new infections follow real infection dynamics, periodically driven by high contact rates on weekdays and lower ones on weekends. Usually, people assume that the weekly wave patterns are just reporting artefacts and that weekly mean values are the finest usable data. One focus of the paper is to quantify the increase in detection rate due to the introduction of rapid testing in schools. For this purpose, we compare federal states that differ in the start of school tests and Easter holidays. There is a clear temporal correlation with the identified detection rates. Finally, we compare the effect of the individual non-pharmaceutical measures by replacing one by one the fitted rates of detection, vaccination and contacts by neutral ones. The increase in the simulated number of actually infected persons measures the effect of the measure ignored.

Comprehensive treatment and analysis of Fishpond sediments as a source of organic fertilizers

Agricultural fertilizers are essential to enhance proper growth and crop yield. Chemical fertilizers endanger ecosystems, soil, plants, and animal and human lives. This has increased interest in biofertilizers which are products that contain living microorganisms or natural compounds derived from organisms such as bacteria, fungi, and algae that improve soil chemical and biological properties, stimulate plant growth and restore soil fertility. This study aimed to use natural coagulants in the management of fishpond sediment for the potential recovery of nutrients that can be utilized in crop production as organic fertilizers. Fishpond wastewater and sediment were collected, treated with natural, powdered Moringa oleifera seed coagulant, and dried. Greenhouse evaluation of the dried sediment was carried out using maize seeds on soil primed with the sediment. Growth parameters such as plant height, leaf length, root length, plant weight, and percentage yield were determined. The nitrate, total nitrogen, phosphate, and total phosphorus content of the organic fertilizer were also determined. Plant height in maize showed a 35.8-44.4 % increase in fertilized maize and a 20.3-39.3 % increase in the leaf length. There was a 38.8-66 % increase in root length and a 23.9-36.5 % increase in plant weight in fertilized maize. The nutritive component was 59.80 mg/Kg, 655.56 mg/Kg, 103.87 mg/Kg, and 426.60 mg/Kg respectively. For a period of 21 days of active growth, the growth rate of maize primed with organic fertilizer was 0.93 cm/day with a total yield of 100 % while the control was 0.68 cm/day with a total yield of 50 %. A dried biofertilizer that can improve plant growth, weight, and percentage yield without having to deal with smelly manure looks promising and seems to be a better alternative to explore in agriculture.

Whats next? Forecasting scientific research trends

Scientific research trends and interests evolve over time. The ability to identify and forecast these trends is vital for educational institutions, practitioners, investors, and funding organizations. In this study, we predict future trends in scientific publications using heterogeneous sources, including historical publication time series from PubMed, research and review articles, pre-trained language models, and patents. We demonstrate that scientific topic popularity levels and changes (trends) can be predicted five years in advance across 40 years and 125 diverse topics, including life-science concepts, biomedical, anatomy, and other science, technology, and engineering topics. Preceding publications and future patents are leading indicators for emerging scientific topics. We find the ratio of reviews to original research articles informative for identifying increasing or declining topics, with declining topics having an excess of reviews. We find that language models provide improved insights and predictions into temporal dynamics. In temporal validation, our models substantially outperform the historical baseline. Our findings suggest that similar dynamics apply across other scientific and engineering research topics. Challenges and opportunities to computationally deconvolve heterogeneous tissue with varying cell sizes using single cell RNA-sequencing datasets

Deconvolution of cell mixtures in "bulk" transcriptomic samples from homogenate human tissue is important for understanding the pathologies of diseases. However, several experimental and computational challenges remain in developing and implementing transcriptomics-based deconvolution approaches, especially those using a single cell/nuclei RNA-seq reference atlas, which are becoming rapidly available across many tissues. Notably, deconvolution algorithms are frequently developed using samples from tissues with similar cell sizes. However, brain tissue or immune cell populations have cell types with substantially different cell sizes, total mRNA expression, and transcriptional activity. When existing deconvolution approaches are applied to these tissues, these systematic differences in cell sizes and transcriptomic activity confound accurate cell proportion estimates and instead may quantify total mRNA content. Furthermore, there is a lack of standard reference atlases and computational approaches to facilitate integrative analyses, including not only bulk and single cell/nuclei RNA-seq data, but also new data modalities from spatial -omic or imaging approaches. New multi-assay datasets need to be collected with orthogonal data types generated from the same tissue block and the same individual, to serve as a "gold standard" for evaluating new and existing deconvolution methods. Below, we discuss these key challenges and how they can be addressed with the acquisition of new datasets and approaches to analysis.

A new sulfur bioconversion process development for energy- and space-efficient secondary wastewater treatment

Harvesting organic matter from wastewater is widely applied to maximize energy recovery; however, it limits the applicability of secondary treatment for acceptable effluent discharge into surface water bodies. To turn this bottleneck issue into an opportunity, this study developed oxygen-induced thiosulfatE production duRing sulfATe reductiOn (EARTO) to provide an efficient electron donor for wastewater treatment. Typical pretreated wastewater was synthesized with chemical oxygen demand of 110 mg/L, sulfate of 50 mg S/L, and varying dissolved oxygen (DO) and was fed into a moving-bed biofilm reactor (MBBR). The MBBR was operated continuously with a short hydraulic retention time of 40 min for 349 days. The formation rate of thiosulfate reached 0.12-0.18 g S/(m2.d) with a high produced thiosulfate-S/TdS-S ratio of 38-73% when influent DO was 2.7-3.6 mg/L. The sludge yield was 0.23-0.29 gVSS/gCOD, much lower than it was in conventional activated sludge processes. Then, batch tests and metabolism analysis were conducted to confirm the oxygen effect on thiosulfate formation, characterize the roles of sulfate and microbial activities, and explore the mechanism of oxygen-induced thiosulfate formation in ERATO. Results examined that oxygen supply promoted the thiosulfate-Sproduced/TdS-Sproduced ratio from 4% to 24-26%, demonstrated that sulfate and microbial activities were critical for thiosulfate production, and indicated that oxygen induces thiosulfate formation through two pathways: 1) direct sulfide oxidation, and 2) indirect sulfide oxidation, sulfide is first oxidized to S0 (dominant) which then reacts with sulfite derived from oxygen-regulated biological sulfate reduction. The proposed compact ERATO process, featuring high thiosulfate production and low sludge production, supports space- and energy-efficient secondary wastewater treatment.

Quantitative Methods for Optimizing Patient Outcomes in Liver Transplantation

Liver transplantation continues to be the gold standard for treating patients with end-stage liver diseases. However, despite the huge success of liver transplantation in improving patient outcomes, long term graft survival continues to be a major problem. The current clinical practice in the management of liver transplant patients is centered around immunosuppressive multidrug regimens. Current research has been focusing on phenotypic personalized medicine as a novel approach in the optimization of immunosuppression, a regressional math modeling focusing on individual patient dose and response using specific markers like transaminases. A prospective area of study includes the development of a mechanistic computational math modeling for optimizing immunosuppression to improve patient outcomes and increase long-term graft survival by exploring the intricate immune/drug interactions to help us further our understanding and management of medical problems like transplants, autoimmunity, and cancer therapy. Thus, by increasing long-term graft survival, the need for redo transplants will decrease, which will free up organs and potentially help with the organ shortage problem promoting equity and equal opportunity for transplants, as well as decreasing the medical costs associated with additional testing and hospital admissions. Although long-term graft survival remains challenging, computational and quantitative methods have led to significant improvements. In this article, we review recent advances and remaining opportunities. We focus on the following topics: donor organ availability and allocation with a focus on equity, monitoring of patient and graft health, and optimization of immunosuppression dosing.

The Brain Tumor Segmentation (BraTS-METS) Challenge 2023: Brain Metastasis Segmentation on Pre-treatment MRI

The translation of AI-generated brain metastases (BM) segmentation into clinical practice relies heavily on diverse, high-quality annotated medical imaging datasets. The BraTS-METS 2023 challenge has gained momentum for testing and benchmarking algorithms using rigorously annotated internationally compiled real-world datasets. This study presents the results of the segmentation challenge and characterizes the challenging cases that impacted the performance of the winning algorithms. Untreated brain metastases on standard anatomic MRI sequences (T1, T2, FLAIR, T1PG) from eight contributed international datasets were annotated in stepwise method: published UNET algorithms, student, neuroradiologist, final approver neuroradiologist. Segmentations were ranked based on lesion-wise Dice and Hausdorff distance (HD95) scores. False positives (FP) and false negatives (FN) were rigorously penalized, receiving a score of 0 for Dice and a fixed penalty of 374 for HD95. Eight datasets comprising 1303 studies were annotated, with 402 studies (3076 lesions) released on Synapse as publicly available datasets to challenge competitors. Additionally, 31 studies (139 lesions) were held out for validation, and 59 studies (218 lesions) were used for testing. Segmentation accuracy was measured as rank across subjects, with the winning team achieving a LesionWise mean score of 7.9. Common errors among the leading teams included false negatives for small lesions and misregistration of masks in space. The BraTS-METS 2023 challenge successfully curated well-annotated, diverse datasets and identified common errors, facilitating the translation of BM segmentation across varied clinical environments and providing personalized volumetric reports to patients undergoing BM treatment.
