Rethinking Clinical Relevance in Chest X-ray Machine Learning: How Evaluation References Define Performance

Chest X-ray (CXR) machine learning relies heavily on automated evaluation using reference standards that aim to approximate clinical judgment. However, commonly used report-derived labels for pathology classification or generic image quality metrics for reconstruction may not reliably reflect clinical judgment. We systematically investigate how evaluation-reference choices affect model performance and ranking in both pathology classification and image quality assessment (IQA). To enable controlled comparison across evaluation references, we collected paired expert image- and report-derived labels for thoracic findings from a clinical cohort at Cambridge University Hospitals (CUH) and curated a subset of the public MIMIC-CXR dataset, along with expert ratings of diagnostic image quality. We show that for supervised image classifiers (ResNet, DenseNet), several zero-shot and fine-tuned vision-language models (e.g., MedKLIP, GLoRIA, and ConVIRT), changing the label source leads to substantial differences not only in performance estimates but also in model rankings. In parallel, alignment of IQA measures with expert judgment depends heavily on the choice of measure, and commonly used IQA metrics such as SSIM and PSNR often fail to align with expert assessments of diagnostic usability. Our results demonstrate that evaluation choices are crucial: they can determine which models and methods appear best and are therefore selected for further development or deployment. The selection of evaluation references should therefore be treated as a central component of clinical validity in CXR machine learning, and justified with respect to the pathology, imaging task, and intended downstream clinical use.

Pair correlation statistics for dynamical systems

We study the pair correlation statistics of orbits generated by maps on the interval. We show that under suitable mixing and multifractal assumptions, the pair correlation statistics of an orbit will almost surely exhibit the same asymptotic behaviour as a suitable sequence of i.i.d. random variables. We will also show that, under suitable hypotheses, the pair correlation statistics defined by two orbits will almost surely exhibit the same behaviour as two suitable sequences of i.i.d. random variables. Specific dynamical systems to which our results apply to include Gibbs-Markov maps and the Gauss map. We also give an example of a slowly mixing system for which the pair correlation statistics of an orbit almost surely behave distinctly to an i.i.d. sequence.

Self-similar and self-conformal measures with slow Fourier decay

Given any function $\phi \colon [0,\infty)\to (0,1]$ satisfying $\lim_{\xi\to\infty}\phi(\xi) = 0$, we prove the existence of i) self-similar measures and ii) nonlinear $C^{\infty}$ self-conformal measures which are Rajchman and whose Fourier transform $\widehat{\mu}$ satisfies \[ \limsup_{\xi\to\infty}\frac{|\widehat{\mu}(\xi)|}{\phi(\xi)}>0.\] Moreover, we derive new sufficient conditions for a self-conformal measure to be Rajchman, and construct an explicit self-similar measure $\mu$ such that $\mu$ almost every $x$ is normal in base $10$ but the sequence $(10^{n}x \mod 1)_{n=1}^{\infty}$ equidistributes extremely slowly.

A quantitative framework for sets of exact approximation order by rational numbers

In this paper we study a quantitative notion of exactness within Diophantine approximation. Given $\Psi:(0,\infty)\to (0,\infty)$ and $\omega:(0,\infty)\to (0,1)$ satisfying $\lim_{q\to\infty}\omega(q)=0$, we study the set of points, which we call $E(\Psi,\omega)$, that are $\Psi$-well approximable but not $\Psi(1-\omega)$-well approximable. We prove results on the cardinality and dimension of $E(\Psi,\omega)$. In particular we obtain the following general statements: (i) For any $\omega:(0,\infty)\to (0,1)$ and $\tau>2$ there exists $\Psi:(0,\infty)\to (0,\infty)$ such that $\lim_{q\to\infty}\frac{-\log \Psi(q)}{\log q}=\tau$ and $E(\Psi,\omega)\neq\emptyset.$ (ii) Under natural monotonicity assumptions on $\Psi$ and $\omega,$ we prove that if $\omega$ decays to zero sufficiently slowly (in a way that depends upon $\Psi$) then $E(\Psi,\omega)$ is uncountable. Moreover, under further natural assumptions on $\Psi$ we can calculate the Hausdorff dimension of $E(\Psi,\omega)$. Our main result demonstrates a new threshold for the behaviour of $E(\Psi,\omega)$. A particular instance of this threshold is illustrated by considering functions of the form $\Psi_{\tau}(q)=q^{-\tau}$ when $\tau\in \mathbb{N}_{\geq 3}$. For these functions we prove the following: (iii) If $\omega(q)= Cq^{-\tau(\tau-1)}$ for some sufficiently large $C$ or $\omega(q)=q^{-\tau'}$ for some $\tau'<\tau(\tau-1),$ then $E(\Psi_{\tau},\omega)$ is uncountable and we calculate its Hausdorff dimension. (iv) If $\omega(q)< cq^{-\tau(\tau-1)}$ for some $c\in (0,1)$ for all $q$ sufficiently large then $E(\Psi_{\tau},\omega)=\emptyset.$

Distinct dimensions for attractors of bi-Lipschitz iterated function systems

In this paper, we construct an iterated function system on the line consisting of two bi-Lipschitz contractions whose attractor has distinct lower, Hausdorff, lower box, upper box, and Assouad dimensions, thereby providing negative answers to certain folklore questions. Furthermore, as a by-product of our study of bi-Lipschitz IFSs, we construct IFSs within this family that exhibit interesting fractal behaviour. In particular, we prove the following two statements: (i) There exists a bi-Lipschitz IFS for which the pushforward of any ergodic measure with positive entropy is not exact dimensional; (ii) There exists a bi-Lipschitz IFS whose attractor has empty interior yet positive Lebesgue measure.

On the Fourier transform of random Bernoulli convolutions

We investigate random Bernoulli convolutions, namely, probability measures given by the infinite convolution \[ \mu_\omega = \mathop{\circledast}_{k=1}^{\infty} \left( \frac{\delta_0 + \delta_{\lambda_1 \lambda_2 \ldots \lambda_{k-1} \lambda_k}}{2} \right), \] where $\omega=(\lambda_k)$ is a sequence of i.i.d. random variables each following the uniform distribution on some fixed interval. We study the regularity of these measures and prove that when $\exp\mathbb{E}\left( \log \lambda_1\right)>\frac{2}{\pi}, $ the Fourier transform $\widehat{\mu}_\omega$ is an $L^{1}$ function almost surely. This in turn implies that the corresponding random self-similar set supporting $\mu_{\omega}$ has non-empty interior almost surely. This improves upon a previous bound due to Peres, Simon and Solomyak. Furthermore, under no assumptions on the value of $\exp \mathbb{E}(\log \lambda_1), $ we prove that $\widehat \mu_\omega$ will decay to zero at a polynomial rate almost surely.

On finitely many base $q$ expansions

Given some integer $m \geq 3$, we find the first explicit collection of countably many intervals in $(1,2)$ such that for any $q$ in one of these intervals, the set of points with exactly $m$ base $q$ expansions is nonempty and moreover has positive Hausdorff dimension. Our method relies on an application of a theorem proved by Falconer and Yavicoli, which guarantees that the intersection of a family of compact subsets of $\mathbb{R}^d$ has positive Hausdorff dimension under certain conditions.

Disintegration results for fractal measures and applications to Diophantine approximation

In this paper we prove disintegration results for self-conformal measures and affinely irreducible self-similar measures. The measures appearing in the disintegration resemble self-conformal/self-similar measures for iterated function systems satisfying the strong separation condition. As an application of our results, we prove the following Diophantine statements: 1. Using a result of Pollington and Velani, we show that if $\mu$ is a self-conformal measure in $\mathbb{R}$ or an affinely irreducible self-similar measure, then there exists $\alpha>0$ such that for all $\beta>\alpha$ we have $$\mu\left(\left\{\mathbf{x}\in \mathbb{R}^{d}:\max_{1\leq i\leq d}|x_{i}-p_i/q|\leq \frac{1}{q^{\frac{d+1}{d}}(\log q)^{\beta}}\textrm{ for i.m. }(p_1,\ldots,p_d,q)\in \mathbb{Z}^{d}\times \mathbb{N}\right\}\right)=0.$$ 2. Using a result of Kleinbock and Weiss, we show that if $\mu$ is an affinely irreducible self-similar measure, then $\mu$ almost every $\mathbf{x}$ is not a singular vector.

Can Rule-Based Insights Enhance LLMs for Radiology Report Classification? Introducing the RadPrompt Methodology

Developing imaging models capable of detecting pathologies from chest X-rays can be cost and time-prohibitive for large datasets as it requires supervision to attain state-of-the-art performance. Instead, labels extracted from radiology reports may serve as distant supervision since these are routinely generated as part of clinical practice. Despite their widespread use, current rule-based methods for label extraction rely on extensive rule sets that are limited in their robustness to syntactic variability. To alleviate these limitations, we introduce RadPert, a rule-based system that integrates an uncertainty-aware information schema with a streamlined set of rules, enhancing performance. Additionally, we have developed RadPrompt, a multi-turn prompting strategy that leverages RadPert to bolster the zero-shot predictive capabilities of large language models, achieving a statistically significant improvement in weighted average F1 score over GPT-4 Turbo. Most notably, RadPrompt surpasses both its underlying models, showcasing the synergistic potential of LLMs with rule-based models. We have evaluated our methods on two English Corpora: the MIMIC-CXR gold-standard test set and a gold-standard dataset collected from the Cambridge University Hospitals.

Fourier Decay from $L^2$-Flattening

We develop a unified approach for establishing rates of decay for the Fourier transform of a wide class of dynamically defined measures. Among the key features of the method is the systematic use of the $L^2$-flattening theorem obtained in \cite{Khalil-Mixing}, coupled with non-concentration estimates for the derivatives of the underlying dynamical system. This method yields polylogarithmic Fourier decay for Diophantine self-similar measures, and polynomial decay for Patterson-Sullivan measures of convex cocompact hyperbolic manifolds, Gibbs measures associated to non-integrable $C^2$ conformal systems, as well as stationary measures for carpet-like non-conformal iterated function systems. Applications include essential spectral gaps on convex cocompact hyperbolic manifolds, fractal uncertainty principles, and equidistribution properties of typical vectors in fractal sets.

On the strong separation condition for self-similar iterated function systems with random translations

Given a self-similar iterated function system $\Phi=\{ \phi_i(x)=\rho_i O_i x+t_i \}_{i=1}^m$ acting on $\mathbb{R}^d$, we can generate a parameterised family of iterated function systems by replacing each $t_i$ with a random vector in $\mathbb{R}^d$. In this paper we study whether a Lebesgue typical member of this family will satisfy the strong separation condition. Our main results show that if the similarity dimension of $\Phi$ is sufficiently small, then a Lebesgue typical member of this family will satisfy the strong separation condition.

Polynomial Fourier decay for fractal measures and their pushforwards

We prove that the pushforwards of a very general class of fractal measures $\mu$ on $\mathbb{R}^d$ under a large family of non-linear maps $F \colon \mathbb{R}^d \to \mathbb{R}$ exhibit polynomial Fourier decay: there exist $C,\eta>0$ such that $|\widehat{F\mu}(\xi)|\leq C|\xi|^{-\eta}$ for all $\xi\neq 0$. Using this, we prove that if $\Phi = \{ \varphi_a \colon [0,1] \to [0,1] \}_{a \in \mathcal{A}}$ is an iterated function system consisting of analytic contractions, and there exists $a \in \mathcal{A}$ such that $\varphi_a$ is not an affine map, then every non-atomic self-conformal measure for $\Phi$ has polynomial Fourier decay; this result was obtained simultaneously by Algom, Rodriguez Hertz, and Wang. We prove applications related to the Fourier uniqueness problem, Fractal Uncertainty Principles, Fourier restriction estimates, and quantitative equidistribution properties of numbers in fractal sets.

On the cardinality and dimension of the slices of Okamoto's functions

The graphs of Okamoto's functions, denoted by $K_q$, are self-affine fractal curves contained in $[0,1]^2$, parameterised by $q \in (1,2)$. In this paper we consider the cardinality and dimension of the intersection of these curves with horizontal lines. Our first theorem proves that if $q$ is sufficiently close to $2$, then $K_q$ admits a horizontal slice with exactly three elements. Our second theorem proves that if a horizontal slice of $K_q$ contains an uncountable number of elements then it has positive Hausdorff dimension provided $q$ is in a certain subset of $(1,2)$. Finally, we prove that if $q$ is a $k$-Bonacci number for some $k \in \mathbb{N}_{\geq 3}$, then the set of $y \in [0,1]$ such that the horizontal slice at height $y$ has $(2m+1)$ elements has positive Hausdorff dimension for any $m \in \mathbb{N}$. We also show that, under the same assumption on $q$, there is some horizontal slice whose cardinality is countably infinite.

Spectral gaps and Fourier dimension for self-conformal sets with overlaps

We prove a uniform spectral gap for complex transfer operators near the critical line associated to overlapping $C^2$ iterated function systems on the real line satisfying a Uniform Non-Integrability (UNI) condition. Our work extends that of Naud (2005) on spectral gaps for nonlinear Cantor sets to allow overlaps. The proof builds a new method to reduce the problem of the lack of Markov structure to average contraction of products of random Dolgopyat operators. This approach is inspired by a disintegration technique developed by Algom, the first author and Shmerkin in the study of normal numbers. As a consequence of the method of the second author and Stevens, our spectral gap result implies that the Fourier transform of any non-atomic self-conformal measure decays to zero at a polynomial rate for any $C^{2}$ iterated function system satisfying UNI. This latter result leads to Fractal Uncertainty Principles with arbitrary overlaps.

Quantitative recurrence and the shrinking target problem for overlapping iterated function systems

In this paper we study quantitative recurrence and the shrinking target problem for dynamical systems coming from overlapping iterated function systems. Such iterated function systems have the important property that a point often has several distinct choices of forward orbit. As is demonstrated in this paper, this non-uniqueness leads to different behaviour to that observed in the traditional setting where every point has a unique forward orbit. We prove several almost sure results on the Lebesgue measure of the set of points satisfying a given recurrence rate, and on the Lebesgue measure of the set of points returning to a shrinking target infinitely often. In certain cases, when the Lebesgue measure is zero, we also obtain Hausdorff dimension bounds. One interesting aspect of our approach is that it allows us to handle targets that are not simply balls, but may have a more exotic geometry.

Recurrence rates for shifts of finite type

Let $\Sigma_{A}$ be a topologically mixing shift of finite type, let $\sigma:\Sigma_{A}\to\Sigma_{A}$ be the usual left-shift, and let $\mu$ be the Gibbs measure for a H\"{o}lder continuous potential that is not cohomologous to a constant. In this paper we study recurrence rates for the dynamical system $(\Sigma_{A},\sigma)$ that hold $\mu$-almost surely. In particular, given a function $\psi:\mathbb{N}\to \mathbb{N}$ we are interested in the following set $$R_{\psi}=\{{\texttt i}\in \Sigma_{A}:i_{n+1}\ldots i_{n+\psi(n)+1}=i_1\ldots i_{\psi(n)}\textrm{ for infinitely many }n\in\mathbb{N}\}.$$ We provide sufficient conditions for $\mu(R_{\psi})=1$ and sufficient conditions for $\mu(R_{\psi})=0$. As a corollary of these results, we discover a new critical threshold where the measure of $R_{\psi}$ transitions from zero to one. This threshold was previously unknown even in the special case of a non-uniform Bernoulli measure defined on the full shift. The proofs of our results combine ideas from Probability Theory and Thermodynamic Formalism. In our final section we apply our results to the study of dynamics on self-similar sets.

A note on dyadic approximation in Cantor's set

We consider the convergence theory for dyadic approximation in the middle-third Cantor set, $K$, for approximation functions of the form $\psi_{\tau}(n) = n^{-\tau}$ ($\tau \ge 0$). In particular, we show that for values of $\tau$ beyond a certain threshold we have that almost no point in $K$ is dyadically $\psi_{\tau}$-well approximable with respect to the natural probability measure on $K$. This refines a previous result in this direction obtained by the first, third, and fourth named authors (arXiv, 2020).

Approximating elements of the middle third Cantor set with dyadic rationals

Let $C$ be the middle third Cantor set and $\mu$ be the $\frac{\log 2}{\log 3}$-dimensional Hausdorff measure restricted to $C$. In this paper we study approximations of elements of $C$ by dyadic rationals. Our main result implies that for $\mu$ almost every $x\in C$ we have $$\#\left\{1\leq n\leq N:\left|x-\frac{p}{2^n}\right| \leq \frac{1}{n^{0.01}\cdot 2^{n}}\textrm{ for some }p\in\mathbb{N}\right\}\sim 2\sum_{n=1}^{N}n^{-0.01}.$$ This improves upon a recent result of Allen, Chow, and Yu which gives a sub-logarithmic improvement over the trivial approximation rate.
