CardioBench: A Real-World Data Benchmark for Evaluating Large Language Models in Clinically Authentic Cardiovascular Care Scenarios

Background: Most medical large language model (LLM) benchmarks focus on examination knowledge or isolated tasks and may not reflect the longitudinal, multimodal, and safety-critical workflow of cardiovascular care. Objective: To develop CardioBench, a real-world benchmark spanning the cardiovascular care continuum, and assess LLM performance across clinical dimensions and specialist tasks. Methods: CardioBench includes 2,263 items from 13 task-specific datasets derived from de-identified cardiovascular records and examination data. Sixteen cardiology physicians conducted annotation and reference construction, followed by cross-review from two senior cardiologists. Seven LLMs generated 15,841 outputs under standardized zero-shot settings. Open-ended tasks were evaluated using key-point coverage and holistic clinical quality, while CardioEthics was scored by accuracy. Results: GPT-5.4 achieved the highest macro-average (62.55) and item-weighted mean (62.19), followed by Gemini 3.1 Pro (59.95) and Qwen 3.6 27B (59.72). GPT-5.4 ranked first in all three dimensions. CardioAuxReport performed best (86.38), whereas CardioECGRead (17.25) and CardioEthics (17.34) were lowest. The largest gaps between holistic clinical quality and key-point coverage occurred in CardioComm (52.71), CardioEmergRescue (52.05), and CardioTreatPlan (48.80). Conclusions: To our knowledge, CardioBench is the largest real-world, multi-task benchmark for LLM evaluation across the cardiovascular care continuum and offers the broadest coverage of clinically authentic cardiology scenarios reported to date. It provides a rigorous framework for identifying model strengths, clinically important omissions, and priorities for future development.

A Signal-Language Foundation Model for Broad-Spectrum Cardiovascular Assessment from Routine Electrocardiography

Electrocardiography (ECG) is central to cardiovascular care, but conventional AI models are often restricted to common arrhythmias and may generalize poorly across populations or clinically subtle diseases. We developed ECG Contrastive Language-Image Pre-training (ECGCLIP), a signal-language contrastive learning framework that aligns ECG waveforms with expert diagnostic reports. ECGCLIP was pre-trained on 2,837,962 ECG studies from 1,324,856 patients and evaluated on a held-out internal test set plus nine independent external cohorts comprising about 1.5 million ECGs. Evaluation covered 89 downstream tasks, including 45 ECG diagnoses, 39 echocardiographic targets, and 5 rare cardiac diseases, using PRAUC as the primary metric. ECGCLIP consistently improved performance over random initialization and Merl-R18 baselines. On the internal test set, ECGCLIP-R34 achieved strong performance for atrial fibrillation (PRAUC 0.900) and ST-segment elevation myocardial infarction (PRAUC 0.383), with robust generalization across all external cohorts. It also improved low-prevalence and diagnostically elusive diseases, including Ebstein anomaly, constrictive pericarditis, dextrocardia, and cardiac amyloidosis, with internal PRAUC values of 0.253, 0.175, 0.121, and 0.201, respectively. ECGCLIP was data efficient, matching or exceeding full-dataset baseline performance with only 10% of training data. Feature visualization and saliency analysis suggested clinically meaningful representations aligned with established electrocardiographic criteria. These findings indicate that large-scale ECG-report contrastive pre-training can expand routine ECG interpretation beyond common arrhythmias toward broad cardiovascular assessment and opportunistic screening of echocardiographic and rare conditions.

Data distribution impacts the performance and generalisability of contrastive learning-based foundation models of electrocardiograms

Contrastive learning is a widely adopted self-supervised pretraining strategy, yet its dependence on cohort composition remains underexplored. We present Contrasting by Patient Augmented Electrocardiograms (CAPE) foundation model and pretrain on four cohorts (n = 5,203,352), from diverse populations across three continents (North America, South America, Asia). We systematically assess how cohort demographics, health status, and population diversity influence the downstream performance for prediction tasks also including two additional cohorts from another continent (Europe). We find that downstream performance depends on the distributional properties of the pretraining cohort, including demographics and health status. Moreover, while pretraining with a multi-centre, demographically diverse cohort improves in-distribution accuracy, it reduces out-of-distribution (OOD) generalisation of our contrastive approach by encoding cohort-specific artifacts. To address this, we propose the In-Distribution Batch (IDB) strategy, which preserves intra-cohort consistency during pretraining and enhances OOD robustness. This work provides important insights for developing clinically fair and generalisable foundation models.

Advancing Embodied Intelligence in Robotic-Assisted Endovascular Procedures: A Systematic Review of AI Solutions

Endovascular procedures have revolutionized vascular disease treatment, yet their manual execution is challenged by the demands for high precision, operator fatigue, and radiation exposure. Robotic systems have emerged as transformative solutions to mitigate these inherent limitations. A pivotal moment has arrived, where a confluence of pressing clinical needs and breakthroughs in AI creates an opportunity for a paradigm shift toward Embodied Intelligence (EI), enabling robots to navigate complex vascular networks and adapt to dynamic physiological conditions. Data-driven approaches, leveraging advanced computer vision, medical image analysis, and machine learning, drive this evolution by enabling real-time vessel segmentation, device tracking, and anatomical landmark detection. Reinforcement learning and imitation learning further enhance navigation strategies and replicate expert techniques. This review systematically analyzes the integration of EI into endovascular robotics, identifying profound systemic challenges such as the heterogeneity in validation standards and the gap between human mimicry and machine-native capabilities. Based on this analysis, a conceptual roadmap is proposed that reframes the ultimate objective away from systems that supplant clinical decision-making. This vision of augmented intelligence, where the clinician's role evolves into that of a high-level supervisor, provides a principled foundation for the future of the field.

Medial Injury/Dysfunction Induced Granulation Tissue Repair is the Pathogenesis of Atherosclerosis

Atherosclerosis, a chronic lesion of vascular wall, remains a leading cause of death and loss of life years. Classical hypotheses for atherosclerosis are long-standing mainly to explain atherogenesis. Unfortunately, these hypotheses may not explain the variation in the susceptibility to atherosclerosis. These issues are controversial over the past 150 years. Atherosclerosis from human coronary arteries was examined and triangle of media was found to be a true portraiture of cells injury in the media, and triangle of intima was a true portraiture of myofibroblast proliferation, extracellular matrix (ECM) secretion, collagen fiber formation and intimal thickening to repair media dysfunction. Myofibroblasts, ECM and lumen (intima)/vasa vasorum (VV) (adventitia) constitute granulation tissue repair. With granulation tissue hyperplasia, lots of collagen fibers (normal or denatured), foam cells and new capillaries formed. Thus, the following theory was postulated: Risk factors induce smooth muscle cells (SMCs) injury/loss, and fibrosis or structure destruction could be developed in the media, which lead to media dysfunction. Media dysfunction prompts disturbed mechanical properties of blood vessels, resulting in bigger pressure buildup in the intima and adventitia. Granulation tissues in the intima/adventitia develop to repair the injured media. Atherosclerosis, stiffening or aneurysm develops depending on media dysfunction severity and granulated tissue repair mode/degree. Nearly all characteristics of clinical atherosclerosis could be ideally interpreted with the theory. We believe that media dysfunction is a key initiator in the pathogenesis of atherosclerosis. It is expected that media dysfunction theory of atherosclerosis, should offer better understanding of the etiology for atherosclerosis.

Hemodynamic Bigger Hydrostatic Pressure Instead of Lower Shear Stress Aggravates Atherosclerosis

The traditional view is that low shear stress of blood flow accelerates atherosclerosis. How does a low shear stress damage the vessel or aggravate atherosclerosis? It is a puzzling question. Furthermore, shear stress of veins is similar to that of Relatively Vulnerable Zones (RVZ) of arteries, while intimal thickening of veins is not obvious as the arteries. There are many places in arterioles with lower shear stress as the decrease of blood velocity, while the thickening of arterioles is not as obvious as that of the medium-sized arteries. These issues make it unconvincing for us to explain atherogenesis with lower shear stress. According to Bernoulli fluid equation (P+1/2\r{ho}v^2+\r{ho}gh=constant), in a closed pipe full of flowing blood, at the same height, the hydrostatic pressure is inversely proportional to the second power of blood velocity. At any point of per unit mass of fluid micro cluster here, the reduction of 1/2\r{ho}v^2 would be converted into P. Therefore, hydrostatic pressure(P) is inversely proportional to v^2 in a very short distance or in the same transection of the artery. When blood micro cluster flows over a very short distance or the same transection of the artery, previous studies did not consider the conversion between 1/2\r{ho}v^2 and P. Therefore, low shear stress aggravates atherosclerosis is an appearance, and the essence is that these areas with smaller blood velocity have much bigger hydrostatic pressure, which aggravates atherosclerosis. This perfectly explains the predisposing sites of atherosclerosis.
