The impact of electronic health records (EHR) data continuity on prediction model fairness and racial-ethnic disparities

Electronic health records (EHR) data have considerable variability in data completeness across sites and patients. Lack of "EHR data-continuity" or "EHR data-discontinuity", defined as "having medical information recorded outside the reach of an EHR system" can lead to a substantial amount of information bias. The objective of this study was to comprehensively evaluate (1) how EHR data-discontinuity introduces data bias, (2) case finding algorithms affect downstream prediction models, and (3) how algorithmic fairness is associated with racial-ethnic disparities. We leveraged our EHRs linked with Medicaid and Medicare claims data in the OneFlorida+ network and used a validated measure (i.e., Mean Proportions of Encounters Captured [MPEC]) to estimate patients' EHR data continuity. We developed a machine learning model for predicting type 2 diabetes (T2D) diagnosis as the use case for this work. We found that using cohorts selected by different levels of EHR data-continuity affects utilities in disease prediction tasks. The prediction models trained on high continuity data will have a worse fit on low continuity data. We also found variations in racial and ethnic disparities in model performances and model fairness in models developed using different degrees of data continuity. Our results suggest that careful evaluation of data continuity is critical to improving the validity of real-world evidence generated by EHR data and health equity.

Enhancing Renal Tumor Malignancy Prediction: Deep Learning with Automatic 3D CT Organ Focused Attention

Accurate prediction of malignancy in renal tumors is crucial for informing clinical decisions and optimizing treatment strategies. However, existing imaging modalities lack the necessary accuracy to reliably predict malignancy before surgical intervention. While deep learning has shown promise in malignancy prediction using 3D CT images, traditional approaches often rely on manual segmentation to isolate the tumor region and reduce noise, which enhances predictive performance. Manual segmentation, however, is labor-intensive, costly, and dependent on expert knowledge. In this study, a deep learning framework was developed utilizing an Organ Focused Attention (OFA) loss function to modify the attention of image patches so that organ patches attend only to other organ patches. Hence, no segmentation of 3D renal CT images is required at deployment time for malignancy prediction. The proposed framework achieved an AUC of 0.685 and an F1-score of 0.872 on a private dataset from the UF Integrated Data Repository (IDR), and an AUC of 0.760 and an F1-score of 0.852 on the publicly available KiTS21 dataset. These results surpass the performance of conventional models that rely on segmentation-based cropping for noise reduction, demonstrating the frameworks ability to enhance predictive accuracy without explicit segmentation input. The findings suggest that this approach offers a more efficient and reliable method for malignancy prediction, thereby enhancing clinical decision-making in renal cancer diagnosis.

Diffusion-based Dynamic Contract for Federated AI Agent Construction in Mobile Metaverses

Mobile metaverses are envisioned as a transformative digital ecosystem that delivers immersive, intelligent, and ubiquitous services through mobile devices. Driven by Large Language Models (LLMs) and Vision-Language Models (VLMs), Artificial Intelligence (AI) agents hold the potential to empower the creation, maintenance, and evolution of mobile metaverses, enabling seamless human-machine interaction and dynamic service adaptation. Currently, AI agents are primarily built upon cloud-based LLMs and VLMs. However, several challenges hinder their efficient deployment, including high service latency and a risk of sensitive data leakage during perception and processing. In this paper, we develop an edge-cloud collaboration-based federated AI agent construction framework in mobile metaverses. Specifically, Edge Servers (ESs), as agent infrastructures, first create agent modules in a distributed manner. The cloud server then integrates these modules into AI agents and deploys them at the edge, thereby enabling low-latency AI agent services for users. Considering that ESs may exhibit dynamic levels of willingness to participate in federated AI agent construction, we design a two-period dynamic contract model to continuously incentivize ESs to participate in agent module creation, effectively addressing the dynamic information asymmetry between the cloud server and ESs. Furthermore, we propose an Enhanced Diffusion Model-based Soft Actor-Critic (EDMSAC) algorithm to effectively generate optimal dynamic contracts. In the algorithm, we apply dynamic structured pruning to DM-based actor networks to enhance denoising efficiency and policy learning performance. Simulation results demonstrate that the EDMSAC algorithm outperforms the DMSAC algorithm by up to $23\%$ in optimal dynamic contract generation.

Channel Gain Map Reconstruction Based on Virtual Scatterer Model

This paper proposes an efficient method for modeling and reconstructing the channel gain map (CGM) based on virtual scatterers. Specifically, we develop a virtual scatterer model to characterize the channel power gain distribution in three-dimensional (3D) space, by capturing the multi-path propagation environment structure and exploiting the angular-domain spatial correlation of scatterer response. In this model, the CGM is represented as a function over a set of tunable parameters for virtual scatterers, including their number, positions, and scatterer response coefficients (SRCs), which can be estimated from a limited number of channel power gain measurements at a given set of locations within the region of interest. This new representation offers a flexible and scalable modeling framework for efficient and accurate CGM reconstruction. Furthermore, we propose a progressive estimation algorithm to acquire the scatterers' parameters. In this algorithm, we gradually increase the number of virtual scatterers to balance the computational complexity and estimation accuracy. In addition, by exploiting the spatial correlation of scatterer response, we propose a Gaussian process regression (GPR)-based inference method to predict the SRCs that cannot be directly estimated. Finally, ray-tracing-based simulation results under realistic physical environments validate the effectiveness of the proposed method, demonstrating that it achieves higher reconstruction accuracy compared to conventional CGM estimation approaches.

Noise-Adaptive Layerwise Learning Rates: Accelerating Geometry-Aware Optimization for Deep Neural Network Training

Geometry-aware optimization algorithms, such as Muon, have achieved remarkable success in training deep neural networks (DNNs). These methods leverage the underlying geometry of DNNs by selecting appropriate norms for different layers and updating parameters via norm-constrained linear minimization oracles (LMOs). However, even within a group of layers associated with the same norm, the local curvature can be heterogeneous across layers and vary dynamically over the course of training. For example, recent work shows that sharpness varies substantially across transformer layers and throughout training, yet standard geometry-aware optimizers impose fixed learning rates to layers within the same group, which may be inefficient for DNN training. In this paper, we introduce a noise-adaptive layerwise learning rate scheme on top of geometry-aware optimization algorithms and substantially accelerate DNN training compared to methods that use fixed learning rates within each group. Our method estimates gradient variance in the dual norm induced by the chosen LMO on the fly, and uses it to assign time-varying noise-adaptive layerwise learning rates within each group. We provide a theoretical analysis showing that our algorithm achieves a sharp convergence rate. Empirical results on transformer architectures such as LLaMA and GPT demonstrate that our approach achieves faster convergence than state-of-the-art optimizers.

Farewell to Item IDs: Unlocking the Scaling Potential of Large Ranking Models via Semantic Tokens

Recent studies on scaling up ranking models have achieved substantial improvement for recommendation systems and search engines. However, most large-scale ranking systems rely on item IDs, where each item is treated as an independent categorical symbol and mapped to a learned embedding. As items rapidly appear and disappear, these embeddings become difficult to train and maintain. This instability impedes effective learning of neural network parameters and limits the scalability of ranking models. In this paper, we show that semantic tokens possess greater scaling potential compared to item IDs. Our proposed framework TRM improves the token generation and application pipeline, leading to 33% reduction in sparse storage while achieving 0.85% AUC increase. Extensive experiments further show that TRM could consistently outperform state-of-the-art models when model capacity scales. Finally, TRM has been successfully deployed on large-scale personalized search engines, yielding 0.26% and 0.75% improvement on user active days and change query ratio respectively through A/B test.

MulVul: Retrieval-augmented Multi-Agent Code Vulnerability Detection via Cross-Model Prompt Evolution

Large Language Models (LLMs) struggle to automate real-world vulnerability detection due to two key limitations: the heterogeneity of vulnerability patterns undermines the effectiveness of a single unified model, and manual prompt engineering for massive weakness categories is unscalable. To address these challenges, we propose \textbf{MulVul}, a retrieval-augmented multi-agent framework designed for precise and broad-coverage vulnerability detection. MulVul adopts a coarse-to-fine strategy: a \emph{Router} agent first predicts the top-$k$ coarse categories and then forwards the input to specialized \emph{Detector} agents, which identify the exact vulnerability types. Both agents are equipped with retrieval tools to actively source evidence from vulnerability knowledge bases to mitigate hallucinations. Crucially, to automate the generation of specialized prompts, we design \emph{Cross-Model Prompt Evolution}, a prompt optimization mechanism where a generator LLM iteratively refines candidate prompts while a distinct executor LLM validates their effectiveness. This decoupling mitigates the self-correction bias inherent in single-model optimization. Evaluated on 130 CWE types, MulVul achieves 34.79\% Macro-F1, outperforming the best baseline by 41.5\%. Ablation studies validate cross-model prompt evolution, which boosts performance by 51.6\% over manual prompts by effectively handling diverse vulnerability patterns.

DTC: A Deformable Transposed Convolution Module for Medical Image Segmentation

In medical image segmentation, particularly in UNet-like architectures, upsampling is primarily used to transform smaller feature maps into larger ones, enabling feature fusion between encoder and decoder features and supporting multi-scale prediction. Conventional upsampling methods, such as transposed convolution and linear interpolation, operate on fixed positions: transposed convolution applies kernel elements to predetermined pixel or voxel locations, while linear interpolation assigns values based on fixed coordinates in the original feature map. These fixed-position approaches may fail to capture structural information beyond predefined sampling positions and can lead to artifacts or loss of detail. Inspired by deformable convolutions, we propose a novel upsampling method, Deformable Transposed Convolution (DTC), which learns dynamic coordinates (i.e., sampling positions) to generate high-resolution feature maps for both 2D and 3D medical image segmentation tasks. Experiments on 3D (e.g., BTCV15) and 2D datasets (e.g., ISIC18, BUSI) demonstrate that DTC can be effectively integrated into existing medical image segmentation models, consistently improving the decoder's feature reconstruction and detail recovery capability.

Two-timescale Optimization for Hybrid Mechanically and Electronically Tunable 6DMA Aided Communication

This letter proposes a hybrid mechanically and electronically tunable six-dimensional movable antenna (6DMA) base station (BS) architecture for future wireless communication networks. Such BS consists of multiple antenna arrays that are mechanically movable along a circular rail to adapt to the horizontal user hotspots, and each array is equipped with pattern reconfigurable antennas (PRAs) that are capable of electronically switching among a set of specified beam patterns to cater to the instantaneous user channels. The mechanical adjustment provides wide-angle coverage but suffers from slow response, while the electronic tuning enables rapid beam reconfiguration but with limited angular range. To effectively combine their complementary advantages, we propose to jointly design both mechanical and electronic configurations to maximize the average sum-rate of users via a two-timescale optimization approach, in which the array positions are optimized on the long timescale according to large-scale user distribution statistics, and the pattern selection vectors are optimized on the short timescale to enable fast beam alignment based on the instantaneous user locations. An alternating optimization algorithm based on the Monte Carlo sampling method is developed to solve the problem efficiently. Finally, simulation results show that our proposed design achieves significant performance gains over various benchmark schemes.

Graph Smoothing for Enhanced Local Geometry Learning in Point Cloud Analysis

Graph-based methods have proven to be effective in capturing relationships among points for 3D point cloud analysis. However, these methods often suffer from suboptimal graph structures, particularly due to sparse connections at boundary points and noisy connections in junction areas. To address these challenges, we propose a novel method that integrates a graph smoothing module with an enhanced local geometry learning module. Specifically, we identify the limitations of conventional graph structures, particularly in handling boundary points and junction areas. In response, we introduce a graph smoothing module designed to optimize the graph structure and minimize the negative impact of unreliable sparse and noisy connections. Based on the optimized graph structure, we improve the feature extract function with local geometry information. These include shape features derived from adaptive geometric descriptors based on eigenvectors and distribution features obtained through cylindrical coordinate transformation. Experimental results on real-world datasets validate the effectiveness of our method in various point cloud learning tasks, i.e., classification, part segmentation, and semantic segmentation.

The Conformal Laplacian and Positive Scalar Curvature Metrics on Manifolds with Boundary

We give examples of spin $4$-manifolds with boundary $(M,\partial M)$ such that the boundary $\partial M$ has a positive scalar curvature metric which cannot be extended to a positive scalar curvature metric on $M$ with mean convex boundary. These manifolds have the equivalent analytic property that for any metric $g$ on $M$, the conformal Laplacian on $M$ with appropriate boundary conditions and the conformal Laplacian on $\partial M$ cannot both be positive. The obstruction to the positivity of the conformal Laplacians is given by a real-valued $ξ$-invariant associated to the APS theorem for the twisted Dirac operator. We use analytic techniques related to the prescribed scalar curvature problem in conformal geometry to directly treat metrics which are not a product near the boundary.

Near-Field Multi-Cell ISCAP with Extremely Large-Scale Antenna Array

This paper investigates a coordinated multi-cell integrated sensing, communication, and powering (ISCAP) system operating in the electromagnetic near field, where each base station (BS) employs an extremely large-scale antenna array (ELAA) to simultaneously support downlink communication, wireless power transfer (WPT), and environmental sensing. Three categories of communication users (CUs) with different interference cancellation capabilities are considered, and sensing is enabled through a distributed multiple-input multiple-output (MIMO) radar architecture. To address the resulting design challenges, a robust optimization framework is proposed by optimizing the beamforming strategy to maximize the worst-case detection probability over a prescribed sensing region, subject to per-user signal-to-interference-plus-noise ratio (SINR) constraints and energy harvesting requirements at energy receivers (ERs), while explicitly capturing the uncertainty in ER locations. By leveraging semidefinite relaxation (SDR), the original non-convex problem is reformulated as a convex semidefinite program with a provably tight relaxation. Furthermore, a low-complexity maximum ratio transmission (MRT)-based suboptimal scheme is developed, yielding a closed-form solution in the asymptotic regime as the number of antenna elements approaches infinity. Extensive numerical results reveal the fundamental trade-offs among sensing accuracy, communication reliability, and WPT efficiency.

NeRF-VIO: Map-Based Visual-Inertial Odometry with Initialization Leveraging Neural Radiance Fields

A prior map serves as a foundational reference for localization in context-aware applications such as augmented reality (AR). Providing valuable contextual information about the environment, the prior map is a vital tool for mitigating drift. In this paper, we propose a map-based visual-inertial localization algorithm (NeRF-VIO) with initialization using neural radiance fields (NeRF). Our algorithm utilizes a multilayer perceptron model and redefines the loss function as the geodesic distance on \(SE(3)\), ensuring the invariance of the initialization model under a frame change within \(\mathfrak{se}(3)\). The evaluation demonstrates that our model outperforms existing NeRF-based initialization solution in both accuracy and efficiency. By integrating a two-stage update mechanism within a multi-state constraint Kalman filter (MSCKF) framework, the state of NeRF-VIO is constrained by both captured images from an onboard camera and rendered images from a pre-trained NeRF model. The proposed algorithm is validated using a real-world AR dataset, the results indicate that our two-stage update pipeline outperforms MSCKF across all data sequences.

PLK-Calib: Single-shot and Target-less LiDAR-Camera Extrinsic Calibration using Plücker Lines

Accurate LiDAR-Camera (LC) calibration is challenging but crucial for autonomous systems and robotics. In this paper, we propose two single-shot and target-less algorithms to estimate the calibration parameters between LiDAR and camera using line features. The first algorithm constructs line-to-line constraints by defining points-to-line projection errors and minimizes the projection error. The second algorithm (PLK-Calib) utilizes the co-perpendicular and co-parallel geometric properties of lines in Plücker (PLK) coordinate, and decouples the rotation and translation into two constraints, enabling more accurate estimates. Our degenerate analysis and Monte Carlo simulation indicate that three nonparallel line pairs are the minimal requirements to estimate the extrinsic parameters. Furthermore, we collect an LC calibration dataset with varying extrinsic under three different scenarios and use it to evaluate the performance of our proposed algorithms.

Global-Graph Guided and Local-Graph Weighted Contrastive Learning for Unified Clustering on Incomplete and Noise Multi-View Data

Recently, contrastive learning (CL) plays an important role in exploring complementary information for multi-view clustering (MVC) and has attracted increasing attention. Nevertheless, real-world multi-view data suffer from data incompleteness or noise, resulting in rare-paired samples or mis-paired samples which significantly challenges the effectiveness of CL-based MVC. That is, rare-paired issue prevents MVC from extracting sufficient multi-view complementary information, and mis-paired issue causes contrastive learning to optimize the model in the wrong direction. To address these issues, we propose a unified CL-based MVC framework for enhancing clustering effectiveness on incomplete and noise multi-view data. First, to overcome the rare-paired issue, we design a global-graph guided contrastive learning, where all view samples construct a global-view affinity graph to form new sample pairs for fully exploring complementary information. Second, to mitigate the mis-paired issue, we propose a local-graph weighted contrastive learning, which leverages local neighbors to generate pair-wise weights to adaptively strength or weaken the pair-wise contrastive learning. Our method is imputation-free and can be integrated into a unified global-local graph-guided contrastive learning framework. Extensive experiments on both incomplete and noise settings of multi-view data demonstrate that our method achieves superior performance compared with state-of-the-art approaches.

Multi-Agent Intelligence for Multidisciplinary Decision-Making in Gastrointestinal Oncology

Multimodal clinical reasoning in the field of gastrointestinal (GI) oncology necessitates the integrated interpretation of endoscopic imagery, radiological data, and biochemical markers. Despite the evident potential exhibited by Multimodal Large Language Models (MLLMs), they frequently encounter challenges such as context dilution and hallucination when confronted with intricate, heterogeneous medical histories. In order to address these limitations, a hierarchical Multi-Agent Framework is proposed, which emulates the collaborative workflow of a human Multidisciplinary Team (MDT). The system attained a composite expert evaluation score of 4.60/5.00, thereby demonstrating a substantial improvement over the monolithic baseline. It is noteworthy that the agent-based architecture yielded the most substantial enhancements in reasoning logic and medical accuracy. The findings indicate that mimetic, agent-based collaboration provides a scalable, interpretable, and clinically robust paradigm for automated decision support in oncology.

Native Intelligence Emerges from Large-Scale Clinical Practice: A Retinal Foundation Model with Deployment Efficiency

Current retinal foundation models remain constrained by curated research datasets that lack authentic clinical context, and require extensive task-specific optimization for each application, limiting their deployment efficiency in low-resource settings. Here, we show that these barriers can be overcome by building clinical native intelligence directly from real-world medical practice. Our key insight is that large-scale telemedicine programs, where expert centers provide remote consultations across distributed facilities, represent a natural reservoir for learning clinical image interpretation. We present ReVision, a retinal foundation model that learns from the natural alignment between 485,980 color fundus photographs and their corresponding diagnostic reports, accumulated through a decade-long telemedicine program spanning 162 medical institutions across China. Through extensive evaluation across 27 ophthalmic benchmarks, we demonstrate that ReVison enables deployment efficiency with minimal local resources. Without any task-specific training, ReVision achieves zero-shot disease detection with an average AUROC of 0.946 across 12 public benchmarks and 0.952 on 3 independent clinical cohorts. When minimal adaptation is feasible, ReVision matches extensively fine-tuned alternatives while requiring orders of magnitude fewer trainable parameters and labeled examples. The learned representations also transfer effectively to new clinical sites, imaging domains, imaging modalities, and systemic health prediction tasks. In a prospective reader study with 33 ophthalmologists, ReVision's zero-shot assistance improved diagnostic accuracy by 14.8% across all experience levels. These results demonstrate that clinical native intelligence can be directly extracted from clinical archives without any further annotation to build medical AI systems suited to various low-resource settings.
